Overview

 

The Cardiac Imaging Nurse Navigator position serves adult patient populations that will be treated within Cone Health, on the Moses H. Cone campus. The position acts as the primary resource for staff, patients and families for information and education on non-invasive cardiac imaging. The Cardiac Imaging Navigator works collaboratively with a multi-disciplinary team, which includes; physician cardiac imagers, cardiac CT and MRI staff, referring physicians and schedulers, to ensure that patient care meets established protocols and quality standards. The navigator uses recognized cardiac imaging metrics to continually improve procedural efficiencies, quality patient care and to understand treatment trends. This position will serve as a catalyst for optimal patient care and efficiencies.

Responsibilities

 

 Monitor Cardiac CT and MRI capacity and coordinates patient scheduling in conjunction with schedulers and cardiology physician imagers.Serve as liaison for IP referrals, CT/MRI staff, and cardiac imagers for scheduling and pre-scan protocols. Liaison to the CHMG HeartCare ambulatory clinics and referring providersProvide patient and family education Communicate with patients pre-scan to insure they know pre-scan requirements and scheduled procedure time. Coordinate advanced CT testing, FFR-CT. Ensures results are sent to HeartFlow? as per designated protocol. Follows-up post scans to ensure results are reviewed upon completion and key stakeholders are aware of results within the required timeframes.Patient, staff, and physician resource. Patient and physician point of contact. Participate in multi-disciplinary cardiac imaging team meetings.Ensure continuity of care throughout the patient imaging process. Facilitate discharge planning, coordinating access to other hospital facilities, diagnostics, post discharge appointments. Performs post-discharge phone calls as neededEstablish, review and report quality performance metrics. Stay up to date on CT/MRI imaging strategies to establish and maintain current evidence based care practices. Provide staff education.Assist in the development of scanning protocols in collaboration with physicians and CT/MRI staff.Assist with program development, monitor patient and procedural volume trends to inform/ensure resources are appropriate. Assist with data collection and analytics as needed. Assist with research

Qualifications

 

EDUCATION:Required: Nursing degreePreferred: BSN

 

EXPERIENCE:Required: Minimum of 2 years cardiac nursing care or interventional radiology nursing experiencePreferred: Prior experience with cardiac procedure patients

 

LICENSURE/CERTIFICATION/REGISTRY/LISTING:Current Valid North Carolina Registered Nurse License.ACLSBLS (CPR)-American Red Cross or AHA Healthcare Provider
